This event when Kousei plays his performance while Kaori has passed away after the unsuccessful surgery is a spiritual experience because after when Kousei enters a dream-like state during his performance, of all the random things for Kousei to dream about, he instead dreams about seeing and conversing with none other than Kaori Miyazono the same girl who was having a surgery in the hospital, and the fact that Kousei immediately knows that Kaori's surgery was unsuccessful and that she had passed away long before the funeral takes place is a clear indication that this is a spiritual experience where after Kaori has passed away, she came to contact with Kousei as a spirit before leaving the world to go to Heaven.
