In particular, we explain how adaptive and mutational events can dramatically influence the outcome of antibiotic therapy by altering the mechanisms of influx and efflux of antibiotics. The identification of porins and pumps as major resistance markers has opened new possibilities for the development of novel therapeutic strategies directed specifically against these mechanisms. Over the past decades, we have observed a rapid increase in the number of antibiotic-resistant clinical isolates, together with the low rate of development and introduction of new antimicrobials. This combination represents a serious threat to human medicine, as we could one day be left unarmed against bacterial pathogens.
