This character debuted in an ask blog created by a user named Ask-Dusttale on February 10th, He was a regular Sans who, upon learning of what the human has done, aims to stop their genocidal actions by slaughtering all of the Underground to acquire enough LV to defeat the human. Since his original debut, the character has gained high amounts of popularity, earning it thousands of pieces of fanart, comics, animations and even a full-fledged fangame.

Sans is the main enemy of the fan-game Dusttale: Last Genocide. His battle takes place in 3 Phases, with varying mechanics and different levels of difficulty. Sans frequently switches the Soul between Red to Blue Mode, which forces the Human to jump through gaps between his bones or slamming it against the walls of the Bullet Board. He often combines the latter with Gaster Blasters. As the final attack of Phase 1, he will use normal, light blue and orange bones alongside many Gaster Blasters, before concluding his onslaught with a circle of Blasters and trying to slam the Soul towards the walls where he'll fire more Blasters. In Phase 2, Sans will start to periodically slash the Bullet Board with his sword, generating a straight white beam. A flashing warning will appear in the place he'll strike.

After the player kept on resetting over and over again, doing Genocide, Sans, remembering all of the previous timelines, decides that the only way to stop the human is by killing all of the monsters to grow stronger so he can defeat the human. Sans design closely mirrors his Undertale counterpart. The key traits of his design is his hoodie, which is now covering his head, his blue sneakers, his dusty body, and his colored eyes. His eyes have two main colors, red and cyan. His right eye is cyan with a red outline, while his left eye is smaller and purely red. He is coated in dust from his attempted genocide.
