He is featured initially as the main antagonist of the series' first part, Phantom Blood , and later returns as the main antagonist of the series' third part, Stardust Crusaders , now solely referred to as Dio. The poor son of Dario Brando, an abusive alcoholic who worked Dio's mother to death, Dio first appears as a young misanthrope who, via the power of an ancient stone mask, becomes an immortal vampire. A century after the events of the first part, he resurfaces as the user of the time-stopping Stand named The World. As the series' most prolific villain, his defining trait is his staunch ambition, which develops into a " might makes right " philosophy and a drive for absolute control through absolute power. When beginning the series, Dio was the character that Araki looked forward to drawing the most. He also revealed that he had not thought up a weakness for the character and that it was difficult to come up with a way for Dio to be defeated. In regard to Dio's characterization, Araki said that he was inspired by FBI profiling of serial killers, admitting that in a way he thinks they must be incredibly strong to be able to commit the heinous crimes that they do. How serial killers control their victims through psychological manipulation interested Araki as well, a trait similar to how Dio uses his charisma to ensure his followers to do his bidding. Dio is canonically bisexual. Araki stated in a interview that "[Dio's] sort of a composed character that could go either way. Upon entering the Joestar estate after his father died, Dio wasted no time unequivocally making young Jonathan's life a living hell to take everything from him, including abusing and later killing Jonathan's dog, Danny. Dio underestimated Jonathan's retaliation for forcefully kissing his beloved Erina Pendleton, spending the next seven years feigning to be Jonathan's friend before enacting his plan to steal the Joestar fortune by poisoning George with the same poison he used to kill Dario. When Jonathan investigates the matter for proof, Dio plots to use an ancient Aztec stone mask to murder Jonathan.

Diego brando is eventually allured by and registers for the Steel Ball Run racebecoming one of its more popular contestants due to his celebrity status, diego brando. Diego is particularly vindictive against anyone who gets in his way, and doesn't hesitate to injure or kill them, although he doesn't manifest murderous intents automatically.

This Diego is from a Parallel World who was introduced to the "base world" of the Saint's Corpse by President Funny Valentine after the death of the original. He is a racer from a similar Steel Ball Run Race happening in a parallel world. As a final precaution, Valentine had tasked this Diego with stealing back the Saint's Corpse in his place, should he be defeated by Johnny Joestar. After stealing the Corpse, Alternate Diego ambushes Johnny in the final stage of the "base world" Steel Ball Run Race, once the latter tries to pursue him. He is a Stand User like his original counterpart, but unlike the original, due to the Saint's Corpse only existing in the "root world," does not wield Scary Monsters. The second Diego is virtually identical in appearance to the first.

Ferdinand , featured in Steel Ball Run. Following Ferdinand's death, Diego Brando retains the transformation ability through the power of the Saint's Left Eye [1] and, as a result, develops his own version of the Stand. His hair turns into spikes that end in round, ball-shaped tips on the back of his head. Traces of his checkered shirt are still visible on his chest. After being under the influence of the Stand for a while, Diego is able to turn into a more human-shaped form while still retaining sharp teeth, claws, dinosaur legs, and a tail, with a deranged, mad expression on his face. The dinosaurs created through Scary Monsters are blue and brown in the case of the small ones in all media featuring it colored.

It wears a headpiece covering its face to below the place of its nose, slanting at a steep angle from the base of its forehead to a peak situated above the rear of its head by about half its height, leaving the face of an inverted triangle visible to the front; somewhat similar to the Red Crown of Lower Egypt W. On its back are twin diving cylinders W , connected via short twin ridged cables to the rear of its mask; perhaps secured by thick, ridged or puffed-up straps running from the rear of its waist over both its shoulders to its front. The back of its hands bears the shape of a clock with similar emblems on both its shoulders and hips.
